The Hitpoints cape is a Cape of Accomplishment that can be bought from Surgeon General Tafani in the Emir's Arena hospital after achieving level 99 in the Hitpoints skill. The skillcape and its corresponding hood can be purchased for 99,000 coins. The skillcape has a white colour and red stripes if trimmed.

Like all skill capes, the Hitpoints cape gives +9 to all defensive stats, and also a +4 bonus to prayer if it is trimmed, which requires a separate skill at level 99. The cape is automatically trimmed if this condition is met.

When the Hitpoints cape is equipped, the player's natural Hitpoints regeneration rate is doubled to 2 Hitpoints per minute instead of the natural rate of 1 Hitpoint per minute. This does not stack with the Rapid Heal prayer, but does stack with the regen bracelet to regenerate 4 Hitpoints per minute.

On the mobile and New Official Client the effect is visible in the form of a second blue sector around the player's hitpoints orb. This blue sector is only visible when its cooldown is less than that of the natural regeneration segment (otherwise it will be hidden behind it). The blue regeneration bar is functionally independent from your natural regeneration bar, and cannot be influenced by external factors such as Rapid heal (though Rapid Heal can still be used to offset your natural regeneration to better match that of the blue regeneration bar for optimal healing). Despite not stacking with the Rapid Heal prayer, the prayer causes the blue regeneration bar to completely disappear, though it does not reset.

This item can be stored in the cape rack of a costume room, as part of the Hitpoint cape set, along with the hood. Ultimate Ironmen will be unable to retrieve individual pieces until the full set is stored.

This cape and the Slayer cape are harder to obtain untrimmed than others, as each point of damage dealt in combat grants 1.33 Hitpoints experience and 4 experience in the player's selected attack style, so a player is likely to get a 99 in one of the other combat skills first. A few options for players who want to obtain this cape untrimmed include training with the controlled attack style and gaining Hitpoints experience through Pest Control rewards. In general, balancing all combat training can easily result in getting this cape untrimmed, especially with more focus given to training Ranged and Magic at Maniacal monkeys before any 99s are reached.

The models used during the emote slightly change depending on if your player character is male or female.

As of 1 October 2024, there are 558,685 players that have achieved level 99 in Hitpoints.
